If you dont need a referral then please call for an appointment – if you need a referral – then provider id for Mila Oshman PT is p1832649. Give this to your PCP and they will generate a referral.
Most plans after your first visit you will need authorization through ACN/ OptumHealthcare – we will submit all paperwork for you –
